LICENSE 2021-2022

Memberships / renewals are done at the Secretariat of CA BALMA, between 10 a.m. and 6 p.m., with your complete registration file including:

• The 2021 membership form below;
• A medical certificate dated less than 6 months OR the completed health questionnaire in the case of a renewal license;
• The contribution (cheque) corresponding to your payment (prices below).

PLEASE NOTE: ANY INCOMPLETE FILE WILL NOT BE TAKEN INTO ACCOUNT
